📌 Important Booking Information 📝 New Patients: Please arrive 15 minutes early to complete any necessary forms if they have not been submitted online.
⏳ Cancellations: Kindly provide at least 24 hours’ notice for any cancellations to avoid applicable fees.
💳 Credit Card Requirement: A valid credit card is required to book appointments.
🩺 Extended Health Benefits: We accept most major insurance providers; however, claims may be subject to denial or delays. You are responsible for any remaining or full balance if a claim is not approved.
